We are thrilled to announce that Dr. Elisa Greene, DC, is now offering Department of Transportation (DOT) Physicals at our location. Appointments are available Monday-Thursday, 9:30am-5:00pm for $125 per exam.

What to Expect

A DOT Physical consists of a comprehensive health review by a certified Medical Examiner to ensure commercial drivers are fit to drive safely. The exam consists of a medical history review, vision test, hearing test, vital sign measurement, urinalysis, and evaluation of the abdomen, heart, lungs, nervous system, and musculoskeletal system.

 

 

  • When you arrive, you should have your paperwork already filled out and bring your Commercial Driver's License (CDL), so a copy can be made. If you do not complete the paperwork, you will need to reschedule the exam.

 

  • Please bring a current list of medications, dosage, and prescribing doctor name and contact information.

 

  • If applicable, please bring any lab work and/or additional documentation regarding any health conditions being monitored by your MD.

 

  • Payment will need to be collected before your exam begins.

 

  • During the physical, the healthcare provider will check your vision, hearing, blood pressure, and overall health to ensure you meet DOT requirements.

 

  • Please make sure you bring any hearing aids or glasses needed for driving on the day of your physical.

 

  • A urine analysis will be performed, so be sure you are adequately hydrated.

 

  • After passing the exam, you will receive documentation needed for your employer and a copy for you to have on hand while driving.

 

  • If you do not meet the requirements of the DOT physical set forth by FMCSA, instruction will be given to you by the Medical Examiner. In some cases, you will be asked to follow up with your MD to provide further information about your health condition, and you will have 45 days to report back to the Medical Examiner. Your exam will be in a "Pending" status. Once the additional information is given to the Medical Examiner, the Pending status is lifted, and your exam is complete with no additional charge. If the 45 days lapses with no follow-up information, you will need to schedule an appointment for a full physical examination and will be charged the full amount of $125.
